2005 HIV/AIDS Community Grants
In May 2005, Horizons Foundation awarded HIV/AIDS Community Grants totalling $447,500 to the following organizations. These grants are supported with funds from the San Francisco AIDS Foundation's 2005 AIDS Walk and AIDS Marathon events.
AIDS Community Research Consortium (ACRC) - $15,000
General support for ACRC's culturally appropriate programs that are responsive to the changing needs of those infected with and affected by, HIV/AIDS and Hepatitis C, and support their health and quality of life
AIDS Emergency Fund (AEF) - $10,000
General support for AEF to provide emergency financial assistance to low-income San Franciscans disabled by HIV/AIDS to help cover basic living expenses and avoid homelessness
AIDS Legal Referral Panel (ALRP) - $7,500
General support for ALRP's free and low-cost services to help people living with HIV/AIDS maintain and improve their health by resolving their legal issues

General support for APEB's array of prevention and care-related services to people with HIV/AIDS in Alameda county
Ark of Refuge - $5,000
General support for the Ark's full continuum of HIV/AIDS programs and services for high risk underserved people with a focus on African-American MSM, MSM/F, GMOC, and TG people
Asian Americans for Community Involvement - $7,500
General support for AACI's HIV/AIDS prevention/education program aimed at reducing HIV infection among API immigrants, with particular attention to men who have sex with men
Asian & Pacific Islander Wellness Center - $5,000
General support for a broad range of care and prevention services for Asians and Pacific Islanders living with HIV/AIDS
Bay Area Young (BAY) Positives - $10,000
General support for BAY Positive's peer-based, psychosocial support and outreach services, run by and for HIV+/AIDS diagnosed young people, up to 26 years
Black Coalition on AIDS - $5,000
General support for BCA's transitional housing and long-term housing, case management, HIV prevention, and other services targeted to African-Americans

General support for CAL-PEP's wide range of HIV/AIDS outreach programs, services, and interventions that are designed to be culturally appropriate for the populations served
Center for AIDS Services (AIDS Alliance) - $5,000
General support for services including the congregate meal program, transportation, acupuncture and herbal therapies, mental health therapy, and facilities for homeless people with HIV/AIDS
Community Awareness and Treatment Services (CATS) - $5,000
General support for CATS' care, prevention, and other HIV/AIDS services to homeless and low income individuals who are dually diagnosed with substance abuse and mental illness
Community Health Empowerment - $7,500
General support for programs for people living with HIV/AIDS including needle exchange, Health on Wheels mobile clinic, and referrals to other services
Continuum - $10,000
General support for Continuum's HIV health care program, case management, and other services to at-risk and medically fragile individuals who are living on the city's streets and in SRO hotels
DCARA - Deaf Gay and Lesbian Center - $5,000
General support for the development of HIV awareness and prevention education media targeted specifically to the the deaf and hard of hearing community in the Bay
Diablo Valley AIDS Center - $5,000
General support for the HIV/AIDS-specific food distribution and nutrition education program, and nutritional assistance to low-income individuals and their families in Contra Costa county

Support for EBCLC HIV/AIDS Law Project, serving approximately 3,400 HIV-affected clients for free with benefits advocacy, housing advocacy, future planning, immigration, family law, debt relief, discrimination, and confidentiality related to HIV status, and other basic civil legal services
Gente Latina de Ambiente (GELAAM) - $5,000
General support for the HIV/AIDS programming of GELAAM, a community-based Latino organization for LGBT individuals and their families and friends in San Mateo county
Grupo Pro Latino De San Jose - $7,500
Support for ProLatino's monolingual and bilingual HIV/AIDS prevention and education materials, as well as outreach services to the LGBT Latino community to decrease new HIV infections
HIV Education and Prevention Project of Alameda County (HEPPAC) - $10,000
General support for HEPPAC's needle exchange, food, and related programs to stop the spread of HIV and hepatitis B and C infection among injection drug users and their partners
Horizons Unlimited - $5,000
Support for Horizons Unlimited's HIV/AIDS Education and Early Intervention Program, which provides services for homeless, street, gang-affiliated, intravenous drug users, HIV-positive people and youth
Larkin Street Youth Services - $7,500
Support for comprehensive housing and supportive services to high-risk and HIV-positive homeless youth in San Francisco

Marin AIDS Project (MAP) - $20,000
General support of MAP's comprehensive services for people with HIV/AIDS in Marin county, including needle exchange, care services, case management, and more
Mission Neighborhood Health Center - $7,500
Support for MNHC's HIV Clinic (Clinica Esperanza), which provides multi-disciplinary care, case management, and education services to HIV+ Latinos/as, particularly Spanish speaking immigrants
Needle Exchange Emergency Distribution (N.E.E.D.) - $10,000
General support for syringe exchange, safer-use supplies, and harm reduction education for injection drug users in the East Bay
New Connections- $10,000
Support for psychosocial case management and behavioral healthcare services for low-income people living with HIV or AIDS throughout Contra Costa county
New Leaf: Services For Our Community - $7,500
To support New Leaf's HIV/AIDS-Crystal Meth Education Project, which aims to reduce the HIV infection rate of gay and bisexual men who use stimulants

Support for acupuncture, health education, Chinese herbs, yoga and Qi Gong exercise, meditation classes, and other natural therapies for more that 400 HIV + women and men in San Francisco
San Francisco's Gay Men's Community Initiative - $5,000
General support for a program to mobilize gay men around a range of grassroots community-building and health-promoting activities focused on HIV prevention and awareness

General support for the AIDS Health Project, which helps people cope with the emotional, psychological, and social challenges of HIV disease
Bill Wilson Center - $10,000
Support for Bill Wilson Center's HIV prevention and education work with youth living in Santa Clara county
Women's Community Clinic - $5,000
Support for the WCC Outreach Program, which works with women to increase their knowledge about sexual and reproductive health and HIV prevention
Women Organized to Respond to Life Threatening Diseases (WORLD) - $10,000
General support for WORLD's HIV education, advocacy training, and emotional support programs focused on HIV-positive women and their loved ones
